ST3 Neurology Interview
There is clinical concern that a confused elderly patient may have a cerebral aneurysm. How would you evaluate if the patient was able to consent for a cerebral angiogram?
Think about capacity to make a decision (Mental Capacity Act 2005). Remember that a seemingly unwise decision does not indicate incapacity
• It must be ensured that the patient can UNDERSTAND why the procedure is required, what is involved, and what the risks and potential benefits are.
• The patient must be able to RETAIN and WEIGH UP the information.
• The patient must be able communicate a DECISION.
• Capacity is specific to a situation (i.e. the patient may have the capacity to consent to be washed but not a cerebral angiogram) and time, (i.e. the time course of the disease may influence capacity e.g. a patient may clinically improve and regain capacity).
• Ensure communication aids are utilised to help the patient communicate (e.g. if the patient is deaf, dysarthric or dysphasic all effort should be taken to overcome these disabilities to try to allow the patient to communicate).
